Sliding Fee Discount Program
The sliding fee discount program is available for those not eligible for Health First Colorado (Colorado’s Medicaid) or CHP+. Nominal fees range between $20.00 – $85.00, while lab fees range from $21.00 – $70.00. You may qualify based on the household size and income guidelines by providing CHS with income verification. This program is not health insurance and is only valid at Community Health Services for Family Planning and sexually transmitted infection screening services.
