Sliding Fee Discount Program
The sliding fee discount program is available for those not eligible for Health First Colorado (Colorado’s Medicaid) or CHP+. Nominal fees range between $20.00 – $85.00, while lab fees range from $21.00 – $70.00. You may qualify based on the household size and income guidelines below. This program is not health insurance and is only valid at Community Health Services for Family Planning Services.
